QUAKE Event Survey

This form is designed to help us plan for a proposed Jr. High trip to attend a Youth Encounter “Quake” in the Wisconsin Dells.  The event takes place April 26-28, 2009 and is designed specifically for junior highers with a great speaker, an awesome worship band, and terrific activities all designed to help us grow closer to God.  The faith-filled event is held at the Wilderness Resort which features America’s largest waterpark resort. 

We need your help in planning this potential event so please take a few minutes to fill out this survey.

What is your Family Name?
The Dells are 4+ hrs from Bloomington. How likely are you to let your child out of school early to attend this event? We would leave St. John's at around 2pm.
The estimated cost of the trip is $175 dollars (not including meals). Please share your opinion on the cost with us.
